Do I need a referral form at the time of my visit?
Depending on your insurance company, you may need to bring a referral at the time of your visit. Check with your Primary Care Physician's office. Referrals may be faxed to our billing department at (734) 712-8157
What should I bring with me?
Your insurance card and referral form (if necessary, see above).
HMO referral, if necessary.
Any lab test results, medical records, xrays, etc, pertaining to your past/present medical condition to help your physician and avoid unnecessary repetition of testing.
